The Health and Safety at Work Act 2015 requires Counties Manukau Health (CM Health) to ensure the safety of its employees whilst at work. In order for CM Health to meet its legal obligations, the following health information is required.  

The information provided will enable Occupational Health and Safety Services (OHSS) to:

  • Make recommendations to managers on fitness to work and reasonable modifications in the workplace
  • Establish a baseline for future health monitoring and health protection

CM Health Pre Employment Health Screening Policy requires that all staff complete the process, including those transferring positions within the DHB. Completion of this screening process is a condition of employment and clearance from OHSS is required prior to the commencement of employment at CM Health.


  • This questionnaire will take approximately 15 minutes to complete
  • Your form and documentation will be reviewed by Occupational Health and Safety Services
  • Following an initial review you may be asked to:
    • Carry out tests. Written communication and a laboratory form will be sent to you by email or post.
    • Make an appointment with Occupational Health and Safety Service to see the Occupational Health and Safety Nurse or the Occupational Health and Safety Physician

The overall process including tests, interpretation of results and clearance takes a minimum of three days and up to 10 working days from the time you have the tests. If follow up is required, then the process will take longer.
