Mental Health - Complaints data

I write in response to your Official Information Act request, dated 21 August 2019. You requested the following information:

I request information regarding complaints made to the DHB about its mental health services, including Community Mental Health services (for adults and children), and its mental crisis phone lines (including after-hours). For each complaint received between 1st January 2018 and 21st August 2019, I would like to know:

1. The date
2. The service being complained about
3. A brief description of the complaint
4. Whether the complainant was a child, or an adult
5. The outcome of the complaint (if any).
