I write in response to your Official Information Act request dated 31 July 2019.
You requested the following information:
• Since September 1 2018, the number of serious adverse events related to maternity andbirthing services and the number of suspected/ possible or yet to be investigated seriousadverse events related to maternity and birthing serviceso(i.e events reported as possibly serious adverse events, but that haven't been confirmed assuch yet).
• For each event, please provide a summary of what happened (including whether the babydied or was stillborn), findings (including any linked to resource concerns), recommendationsand follow-up actions.
